Two details make the picture work. A needle is thin and small, so it is hard to see. Hay is the opposite: it is soft, light, the same colour as the needle's hiding place, and there is a lot of it. Even a needle made of shining steel would not catch the eye in a heap of pale straw.

How old is the saying?

The idea is old. Versions of "a needle in a haystack" are often traced back to the 1500s in English, and writers from that period are frequently cited for early uses. Because early printed English changed so much, it is hard to point to one exact first appearance, so treat any single date you read with some caution. What is clear is that the image has been around for several hundred years, which is a long time for a joke about farm work to survive.
